BOOST and YouthPlaces After School Request for Proposal, FY09

The Family League of Baltimore City is soliciting proposals, through an open, competitive, process from organizations who wish receive funds to partner with Baltimore’s After School Strategy to deliver high-quality BOOST and YouthPlace after school programming to Baltimore City youth.

Questions and Answers Forum: A questions and answers forum will be held Tuesday, May 13th, 2008, from 9:00 A.M. until 12:00 a.m. at Clarence "Du" Burns Arena, 1301 S. Ellwood Avenue, Baltimore, MD 21224. The purpose of the questions and answers forum is to provide an opportunity to discuss the RFP in more detail and to answer any questions. Answers provided at the forum will be posted here following the forum.

Forms to be completed and submitted for the RFP:

Proposal Cover Page: Proposal Cover Page After School.pdf
The proposal cover page is to be fully completed by the applicant and should serve as the top page of the completed proposal.

Attachment 1: Principal Letter of Commitment: Please see the Support Materials section below for a sample letter.

Attachment 2: Family League of Baltimore City Budget Form: Annual Budget Form.xls
This is a Microsoft Excel file. Budget form to be filled out and submitted with application (does not count towards narrative page limit). Please be sure that budget form matches costs outlined in the budget narrative and that the required matching costs are reflected in the Other Funding column.

Attachment 3: BLAST Memorandum of Understanding (MOU to be signed and submitted if program plans to use BLAST): BLAST MOU FY09.pdf
This Memorandum of Understanding, between the applicant and the Safe and Sound Campaign, must be signed and submitted for all programs that are proposing to use the BLAST program for their academic component.

Proposal and Contract Affidavit: Affidavit 2009.pdf
This affidavit assures the Family League that they will contract with a vendor that is legally recognized by the State of Maryland. Furthermore, it ensures that the applicant has reviewed and is accepting of the Standard Terms and Conditions.

Sample Principal Letter of Commitment (required for BOOST only - as Guidance for Attachment 1): Sample Principal Letter of Commitment for BOOST Applicants.pdf
BOOST applicants are required to submit with their applicant, a letter of commitment outlining a basic partnership agreement that signed by both the applicant and the principal from their partner school. This sample letter is provided as guidance for this attachment.
